I understand that it is my responsibility to submit all the necessary official and original documents, duly authenticated, at the time of your registration. I understand that the Registrar Office will not issue any official document until the stipulated documents are submitted and financial obligations met. I vow to abide by each of the regulations set in place regarding discipline and academics. Moreover, I hereby authorize the Academic Institution to inform my parent(s)/ or legal guardian of my academic progress, financial situation, and moral conduct, as each case dictates.
